Waiver Release

 

 

I have registered the attached rostered participants in the FUTSAL program sponsored by the Tri-County Futsal (in conjunction with Calvert Soccer Association, St. Mary’s Youth Soccer, Calvert County Department of Recreation and Parks, St. Mary’s County Department of Recreation and Parks, Charles County Capital Clubhouse and/or Charles County Department of Community Services, St. Mary’s College of Maryland, King’s Christian Academy and CSA Fieldhouse).

 

The participants and their families as listed on the team roster I have submitted, agree they will NOT make a claim against, sue, attach the property of or prosecute Tri County Futsal and …(list of all entities) and their employees or volunteers for damages for death, personal injury or property damage which players may sustain as a result of their participation in Futsal activities. This release is intended to discharge in advance Calvert Soccer Association, St. Mary’s Youth Soccer, Calvert County Department of Recreation and Parks, St. Mary’s County Department of Recreation and Parks, Charles County Capital Clubhouse and/or Charles County Department of Community Services, St. Mary’s College of Maryland, King’s Christian Academy and CSA Fieldhouse and their employees or volunteers from and against any and all liability, including for negligent actions, arising out of or connected in any way with participation in the futsal program. The team representative(s) identified in the registration form acknowledge that permission was received from all player’s parents/legal guardians for participation in Tri-County Futsal. Tri-County Futsal and its representatives, their officials, employees, instructors and volunteers are held harmless from any and all liabilities arising from any injuries that might occur during the program.

 

THE ROSTERED PARTICIPANTS UNDERSTAND THAT SPORTS INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPANTS IN SUCH SPORTING ACTIVITIES OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF. KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE THAT MY MINOR CHILD AND I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS MARYLAND SOCCER FOUNDATION, DISCOVERY SPORTS CENTER, MARYLAND SOCCERPLEX AND THEIR AGENTS, SPONSORS AND EMPLOYEES WHO (THROUGH NEGLIGENCE OR CARELESSNESS) MIGHT OTHERWISE BE LIABLE TO ME, MY MINOR CHILD (OR OUR HEIRS OR ASSIGNS) FOR DAMAGES.

 

AS A REPRESENTATIVE FOR THE ROSTERED TEAM, I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR THE ROSTERED PARTICIPANTS AND IS A CONTRACT BETWEEN MYSELF, THE PARTICPANTS AND THEIR FAMILIES AND Calvert Soccer Association, St. Mary’s Youth Soccer, Calvert County Department of Recreation and Parks, St. Mary’s County Department of Recreation and Parks, Charles County Capital Clubhouse and/or Charles County Department of Community Services, St. Mary’s College of Maryland, King’s Christian Academy and CSA Fieldhouse AND I HAVE SIGNED IT OF MY OWN FREE Will.

 

Submission of this waiver provides permission for any and all medical attention necessary to be administered to any rostered participant in the event of an accident, injury, sickness, etc. under the direction of Tri-County Futsal and its representatives.  I also understand that Tri-County Futsal recommends physicals for children participating in the futsal program and requests that a parent or other designated responsible adult attend all events.

Calvert Soccer Association, St. Mary’s Youth Soccer, Calvert County Department of Recreation and Parks, St. Mary’s County Department of Recreation and Parks, Charles County Capital Clubhouse and/or Charles County Department of Community Services, St. Mary’s College of Maryland, King’s Christian Academy and CSA Fieldhouse.
